We value the trust and confidence of the referring physicians in our practice. We are dedicated in providing your patients with excellent, compassionate and innovative care for all their urologic needs. Our providers are highly qualified and trained to treat wide range of urologic and reproductive disorders by combining extensive experience with innovative technologies.
Refer a Patient
Our office usually contacts the referred patient within 24 hrs from the referral date. In most cases we can accommodate the patient on our schedule within a week depending on patient medical needs and preferences. To refer patients please call our office at (623) 512-4390 or Direct Referral Line at (623) 512-4372. Please fill out the Referral Form and fax it at (623) 512-4391.
PLEASE NOTE: In order for us to see provide best patient care and see the referred patient we will need a referral from the primary care physicians (PCP) office. We will not be able to see the patient until a referral is received from the PCP’s office.
Referral Resources
We constantly strive to improve the referral process by developing tools for referral coordinators and physician offices. You can print these files per your needs and hand them to the patients for ease.
Online Referral Forms
We have incorporated secure online referral forms on our website which will allow you to refer patients to us electronically and fax the medical request and paper referral. The information transmitted via this form is secure and encrypted per HIPPA compliance.
How does it work ?
When you submit the online referral request it is processes by our office within 48 hrs. Our office will contact you if we have not received the medical records and paper referral by fax or other information is required. After all the information is received we call the patient to schedule an appointment.
